What is Advance Care Planning?

Advance Care Planning (ACP) allows individuals to reflect on their values, wishes and care preferences, ultimately making a plan for their care. And care comes in many forms. ACP is more than just making decisions about your medical care. It is about planning now so you can rest easier, and spend more time making memories with your loved ones.

Learning from others, especially experts, makes it easier to understand what might be important to include in your plans so that your individual wishes are clear. You may choose to express your wishes simply, or to provide as much detail that suits you and your unique life.
