Life Happens, whether you are ready for it or not.  Join us for a 3-part series that will assist you in preparing for the unexpected.

These sessions will include educational videos followed by interactive conversations led by Sandra Andreychuk, Health Care Ethicist.

 

Session One: Preparing for the unexpected. 

April 21st @6:00 PM EST – 7:30 PM EST

In this session, we will: 

  • Explore the common discomfort in talking about death and dying.
  • Offer practical ways to walk through planning for our future health and personal care needs.
  • Share the benefits of being prepared for the unexpected.
  • Discuss strategies to be prepared and get organized.
  • Provide you with a workbook.

 

Session Two:  Stand up and be heard.  Choosing the right person for the job.

April 28th @6:00 PM EST – 7:30 PM EST

In this session we will:

  • Explore what it means to be a proxy for property, health, and personal care.
  • Understand the responsibilities of a Power of Attorney and Substitute Decision Maker.
  • Describe the risk in not appointing a Power of Attorney for Property and Personal Care.
  • Identify the traits of a good decision-maker (proxy).

 

Session Three: Life happens on its own terms.  Deciding what to do next can be on your terms.

May 5th @6:00 PM EST – 7:30 PM EST

In this session we will:

  • Explore the common phrase “ quality versus quantity of living” and how our opinions can influence our choices in life.
  • Identify common health care treatment options that often cause angst and uncertainty.
  • Discuss the reality of “staying in our own home” and  the various housing options as we age.

 

Advance Care Planning is a process of reflection and communication. It is a time for you to reflect on your values and wishes, and to let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself.

In Ontario, advance care planning (ACP) is

  • Confirming your substitute decision-maker(s) (SDMs); and
  • Communicating your wishes, values, and beliefs about care to help your SDM(s) make health and personal care decisions for you if you become mentally incapable of doing so for yourself.
