Our 10 Bed Residential Hospice
Since 2007 our residential hospice has been home to more than 1927 families. A referral to stay in our residential hospice palliative care program can be made when the prognosis is approximately three months or less. Various factors are taken into consideration to determine admission. Our residential team of PSW’s, RPN’s, MRP’s and RN’s strive to provide whole-person care – body, mind, and spirit – with a multi-disciplinary team who accompany the resident and their family through the course of their journey.
Please note that a referral for admission into our hospice must be made to the Hamilton Brant Haldimand Home and Community Care Support Services through the following ways:
- Your social worker (if in hospital)
- Your health care professional (if at home this would be your Home and Community Care Support Services Care Coordinator)
- Your family doctor can make a referral for you to the Home and Community Care Support Services
